Registration - Physicals

If it’s more convenient for you, print and complete your registration forms before you arrive. If you are electing to use insurance, remember to bring your insurance card. Just select the service or treatment you need. 

Be sure to print all of the appropriate forms. They will be displayed in a new window for
you to print.
