The "Moderate" level is a doctor's visit with one procedure or one order for lab work, or two medical complaints with low complexity. Clinical visits with a doctor that are complex in nature, have more than one procedure or more than two complaints bill as a "Complex" charge, as does urgent care requiring intravenous injections, treatment with sutures, x-rays for injuries, flushing an eye or receiving special medications.
